Vermox (Mebendazole)
Pharmacological action
Anthelminthic drug of a broad spectrum. Most effective against Enterobius vermicularis, Trichuris trichiura, Ascaris lumbricoides, Ancylostoma duodenale, Necator americanus, Strongyloides stercoralis, Taenia spp., Echinococcus granulosus, Echinococcus multilocularis, Trichinella spiralis, Trichinella nativa, Trichinella nelsoni.
Causes irreversible disruption of glucose utilization, depletes glycogen reserves in helminth tissues, inhibits cellular tubulin synthesis, and inhibits ATP synthesis.
Pharmacokinetics
Absorption and distribution
Virtually not absorbed in the intestine. Cmax in blood plasma is usually observed 2-4 hours after administration. After taking the drug in a dose of 100 mg 2 times per day for 3 consecutive days, plasma concentrations of mebendazole and its metabolite (2-amino derivative) do not exceed 0.03 mcg/ml and 0.09 mcg/ml, respectively. The intake of food with high fat content leads to a slight increase in the bioavailability of mebendazole.
Distribution
Binding of mebendazole with blood plasma proteins is 90-95%.
It is distributed unevenly throughout the organs and accumulates in adipose tissue, liver, helminth larvae. Vd is 1-2 l/kg.
Metabolism
It is metabolized in the liver to 2-amino derivative, which has no anthelminthic activity.
Excretion
T1/2 3-6 h. More than 90% of the dose is excreted unchanged through the intestine. The absorbed part (5-10%) is excreted by the kidneys.
Pharmacokinetics in special groups of patients
In patients with liver dysfunction, metabolic disorders, bile outflow obstruction, increased concentrations of mebendazole in blood plasma may be observed.
Contraindications to use
- Hypersensitivity to mebendazole and other components of the drug;
- ulcerative colitis;
- Crohn's disease;
- hepatic impairment;
- Children under 3 years of age (for this dosage form);
- pregnancy;
- Lactation period (breastfeeding);
- lactose intolerance, lactase deficiency, glucose-galactose malabsorption.
Administration during pregnancy and lactation
Pregnancy
Administration of the drug during pregnancy is contraindicated.
Period of breast-feeding
There is no data on whether mebendazole is excreted with breast milk. If it is necessary to apply the drug during lactation, it is recommended to stop breast-feeding.
Administration in liver dysfunction
The drug is contraindicated in liver failure.
Administration in children
The drug is contraindicated in children under 3 years of age.
Indications of the drug Vermox
enterobiasis, ascariasis, ankylostomiasis (ankylostomiasis, necatoriasis), strongyloidiasis, trichocephaliasis, trichinellosis, teniasis, both in monoinvasion and mixed gastrointestinal helminthiasis;
Echinococcosis (when surgical treatment is impossible).
Dosage regimen
Inside with a small amount of water. Tablets may be chewed or swallowed whole. For younger children, the tablet should be crushed before use. Risk is applied only to facilitate breaking the tablet if it is difficult to swallow it whole. Breaking the tablet along the line of the risk does not allow dividing it into equal doses.
During treatment, taking laxatives or compliance with a special diet or not required.
Adults and children over 3 years of age in case of enterobiasis - once for 100 mg (1 tablet). It is recommended to treat all members of the family at the same time. Since very often there is re-infection (reinfection) of Enterobius vermicularis, it is recommended to repeat the treatment in 2-4 weeks.
Adults and children over 14 years old: in case of echinococcosis during the first 3 days - 500 mg 2 times per day, in the next 3 days the dose is increased to 500 mg 3 times per day; further the dose is increased to 1000-1500 mg 3 times per day. The average duration of treatment of echinococcosis caused by Echinococcus granulosus is 4-6 weeks, that of Echinococcus multilocularis - up to 2 years.
In trichinellosis - in day 1 - 200-300 mg 3 times a day, in day 2 - 200-300 mg 4 times a day, and from day 3 to 14 - 500 mg 3 times a day.
Adults and children over 3 years of age (regardless of body weight and age) with ascariasis, trichocephaliasis, ankylostomiasis, necatorasis and mixed helminth infections: 200 mg/day (1 tablet in the morning and 1 tablet in the evening); treatment course 3 days.
For adults with shadeniosis, strongyloidiasis: 400 mg/day (2 tablets in the morning and 2 tablets in the evening); the course of treatment is 3 days. Children over 3 years of age - 200 mg/day (1 tablet in the morning and 1 tablet in the evening); treatment course is 3 days.
