DRUG TITLE

Symmetrel (Amantadine)

Pharmacological action

Antiparkinsonian and antiviral drug, tricyclic symmetric adamantanamine.


It blocks glutamate NMDA-receptors and thus reduces excessive stimulating effect of cortical glutamate neurons on neostriatum, which develops against the background of dopamine insufficiency. Amantadine inhibits NMDA-receptors of substantia nigra neurons, reduces Ca2+ influx into them, which reduces the possibility of destruction of these neurons. Affects stiffness (rigidity and bradykinesia) to a greater extent.


Inhibits the penetration of influenza A virus into the cell.


Pharmacokinetics

After oral administration, it is completely absorbed from the gastrointestinal tract. Cmax in blood plasma is reached after 5 hours. It is excreted in the urine. T1/2 of amantadine sulfate is 12-13 hours, in contrast to amantadine hydrochloride T1/2 of which is 30 hours.

Contraindications to use

Liver failure, chronic renal failure, psychosis (including history), thyrotoxicosis, epilepsy, closed-angle glaucoma, prostatic hyperplasia, arterial hypotension, chronic heart failure stage II-III, agitation, trimester of pregnancy, lactation, simultaneous use of triamterene and hydrochlorothiazide, hypersensitivity to amantadine.

Administration during pregnancy and lactation

During pregnancy, use of amantadine is possible only under strict indications and under medical supervision. It is contraindicated in the first trimester of pregnancy.


In case of necessity of application during lactation it is necessary to decide on stopping of breast feeding.


Administration in patients with liver dysfunction

Contraindication: liver failure.

Administration in impaired renal function

It is contraindicated in chronic renal failure. Caution is advised when using in patients with kidney dysfunction.

Administration in elderly patients

Use with caution in elderly patients.

Indications of active substances of the drug Amantadine

Parkinson's disease, parkinsonism syndrome (rigidity, tremor, hypokinesia).


Neuralgia in shingles caused by Varicella zoster virus.


Prevention (including in combination with vaccination) and treatment of Influenza A.


Dosing regimen

The optimal dosing regimen is determined by the doctor. You should strictly follow the compliance of the dosage form used for a particular drug indications for use and the dosing regimen.

For oral administration a single dose is 100-200 mg.


V/v as an infusion of 200 mg for 3 hours.


The frequency and duration of use depend on the indication, the patient's response to treatment, and the epidemiological situation.


Maximum dose: when administered orally - 600 mg/day.
