Sporanox (Itraconazole)

Pharmacological action

Antifungal agent, triazole derivative. The mechanism of action is associated with the ability to inhibit cytochrome P450-dependent enzymes of susceptible fungi, which leads to disruption of ergosterol synthesis of the cell wall of the fungus. It has a wider spectrum of antifungal activity than ketoconazole. It is active against Aspergillus spp., Blastomyces dermatitidis, Candida, Coccidioides immitis, Cryptococcus neoformans, Epidermophyton, Microsporum, Trichophyton, Histoplasma capsulatum, Malassezia furfur, Paracoccidioides brasiliensis, Sporothrix schenckii.


Pharmacokinetics

Therapeutic concentrations in the skin are maintained for 2-4 weeks after the end of the 4-week course of treatment. Itraconazole is determined in the keratin of nails one week after the start of treatment and persists for 6 months after the end of the 3-month course of therapy.



Contraindications for use

Simultaneous oral administration of terfenadine, astemizole, misolastine, cisapride, dofetilide, quinidine, pimozide, simvastatin, lovastatin, midazolam or triazolam; hypersensitivity to itraconazole.

Usage in pregnancy and lactation

During pregnancy itraconazole is used only in case of systemic mycoses, when the expected effect of therapy exceeds the potential risk to a fetus.


It is recommended to use contraceptives for women of childbearing age during the use of itraconazole.


If it is necessary to use during lactation, it is necessary to consider stopping breastfeeding.


Itraconazole has embryotoxic effect and causes abnormal fetal development in experimental studies.


Administration in liver dysfunction

Itraconazole should not be used in patients with liver disease. However, in cases of extreme necessity in liver cirrhosis, itraconazole is used under control of plasma levels, and the dose is adjusted accordingly.


In case of increased liver transaminases activity, itraconazole is used in cases when the expected benefit of therapy exceeds the possible risk of liver damage.


In case of itraconazole use for more than 1 month, it is necessary to control the liver function.


In case of liver dysfunction during treatment, itraconazole should be stopped.


Administration in impaired renal function

In cases of extreme need with impaired renal function, treatment with itraconazole is carried out under control of plasma levels and with appropriate dose adjustment.


Administration in children

At present time there are not enough data on the use of itraconazole in children.

Indications of active substances of the drug Sporanox (Itraconazole)

Vulvovaginal candidiasis, mycoses of the skin, mouth, eyes, onychomycosis caused by dermatophytes and/or yeasts, systemic mycosis (including systemic aspergillosis, candidiasis, cryptococcosis, histoplasmosis, sporotrichosis, paracoccidioidosis, blastomycosis and other, rarely occurring systemic and tropical mycosis).

Dosing regimen

The optimal dosing regimen is determined by the doctor. It should be strictly adhered to in accordance with the dosage form of the drug and the dosing regimen.

Orally, 100 mg once a day or 200 mg 1-2 times a day, the duration of treatment depends on the etiology of the disease.
