DRUG TITLE

Sinemet (Carbidopa + Levodopa)

Pharmacological action

Antiparkinsonian drug - a combination of levodopa (dopamine precursor) and carbidopa (peripheral dopa decarboxylase inhibitor). It eliminates hypokinesia, rigidity, tremor, dysphagia, salivation.


The antiparkinsonian effect of levodopa is due to its conversion into dopamine directly in CNS, which leads to replenishment of dopamine deficiency in CNS. Dopamine formed in peripheral tissues does not participate in realization of levodopa antiparkinsonian effect (does not penetrate into CNS) and is responsible for most of levodopa side effects.


Carbidopa is a peripheral dopa decarboxylase inhibitor, reduces dopamine formation in peripheral tissues, which indirectly leads to increased amount of levodopa delivered to CNS.


The optimal combination of levodopa and carbidopa is 4:1 or 10:1.


The effect of this combination is manifested within the first 24 hours from the start of administration, sometimes after the first dose. The full therapeutic effect is achieved within 7 days.


Pharmacokinetics

When taken orally, Levodopa is rapidly absorbed from the gastrointestinal tract. Absorption is 20-30% of the dose, Cmax when taken orally is 2-3 hours. Absorption depends on the rate of gastric contents evacuation and on the pH in the stomach. The presence of food in the stomach slows absorption. Some food amino acids may compete with levodopa for absorption from the intestine and transport through the GEB. Found in large amounts in the small intestine, liver and kidneys, only about 1-3% penetrates the brain. Metabolized in all tissues, mainly by decarboxylation to form dopamine, which does not penetrate the GEB, the metabolites - dopamine, norepinephrine, epinephrine are excreted by the kidneys. T1/2 is about 1 h, when combined with carbidopa it is about 2 h. The unchanged form is excreted by the kidneys (35% within 7 hours) and through the intestine.


After oral administration in healthy individuals and patients with Parkinson's disease in a single dose, Cmax of radioactive isotope-labeled carbidopa in plasma is determined 2-4 hours after intake in healthy individuals and 1.5-5 hours in patients with Parkinson's disease. Urinary and fecal excretion is about the same in both groups. Among the metabolites excreted with the urine, the main ones are alpha-methyl-3-methoxy-4-hydroxyphenylpropionic acid and alpha-methyl-3,4-dihydroxyphenylpropionic acid. They constitute about 14 and 10% of the excreted metabolites, respectively. Two other metabolites are found in smaller amounts. One is identified as 3.4-dihydroxyphenylacetone and the other is tentatively identified as N-methylcarbidopa. The content of each of these substances is no more than 5% of the total metabolites. Unchanged carbidopa is also detected in the urine.


Contraindications to use

Closed-angle glaucoma, melanoma or suspected melanoma and skin diseases of unknown etiology, simultaneous use of non-selective MAO inhibitors, children and adolescents under 18 years of age, hypersensitivity to levodopa and/or carbidopa, pregnancy, breast-feeding period.


Caution


Myocardial infarction with arrhythmia (in anamnesis), CHF, severe lung diseases, including bronchial asthma, epilepsy and other seizures (in anamnesis), erosive and ulcerative gastrointestinal diseases, diabetes and other decompensated endocrine diseases, severe hepatic and/or renal failure, open-angle glaucoma, extrapyramidal reactions induced by this combination.


Administration during pregnancy and lactation

Application during pregnancy is contraindicated.


Breast-feeding should be discontinued if it is necessary to use during lactation.


Administration in patients with hepatic impairment

Caution: severe hepatic insufficiency.

Administration in patients with impaired renal function

Caution: severe renal insufficiency.

Administration in children

It is contraindicated in children under 18 years.

Indications of active ingredients of the drug Synemet

Parkinson's disease, Parkinsonism syndrome (except that caused by antipsychotic drugs) - post-encephalitic, against the background of cerebrovascular diseases, intoxication with toxic substances (including carbon monoxide or manganese).


Dosing regimen

The optimal dosing regimen is determined by the doctor. It should be strictly followed in accordance with the dosage form used for the particular drug indications and the dosing regimen.
