Retin-A (Tretinoin)

Pharmacological action

Retinoid. Structurally similar to vitamin A, it is a natural metabolite of retinol. When administered orally, it inhibits differentiation and growth of altered blood cell clone, including human myeloleukemia cells. The mechanism of action in acute promyelocytic leukemia is probably due to damage caused by tretinoin binding to retinoic acid nuclear receptors.

When applied externally, it enhances cell proliferation in the papillary layer of the skin, reduces cell adhesion involved in the formation of vulgar acne. When treating open acne it helps to even out the surface of the skin without signs of inflammation. When treating closed acne promotes their conversion into open acne or papules, which then heal without scarring after removal of the keratin plug. Prevents the formation of new blackheads.


Tretinoin is a natural metabolite of retinol, which is normally present in the plasma. After oral administration, tretinoin is absorbed from the gastrointestinal tract. Plasma concentrations return to the original endogenous level 7-12 hours after a single dose of 40 mg.

Binding to plasma proteins is high. It does not cumulate.

T1/2 is 0.7 hours. After oral administration of radiolabelled tretinoin about 60% is excreted in the urine and about 30% in the faeces. Metabolites are formed by oxidation and glucuronidation.

With long-term use of tretinoin a decrease in its plasma concentration is observed, which may be due to induction of CYP isoenzymes.

Contraindications to use

Hypersensitivity to tretinoin, pregnancy, breast-feeding (for oral administration), dermal epithelioma in the anamnesis (including family).

For external use: acute inflammatory (including eczematous) skin lesions, wounds, burns.

Using during pregnancy and lactation.

Contraindicated in pregnancy. Women of childbearing age should use effective contraception for at least 1 month before treatment, during therapy and 1 month after cancellation of tretinoin. Use begins on the 2-3rd day of a normal menstrual cycle. 2 weeks before the start of treatment the absence of pregnancy should be confirmed by laboratory tests. Such tests should be carried out once a month.

If it is necessary to use orally during lactation, breastfeeding should be discontinued.

Administration in patients with liver dysfunction

Caution should be exercised when using in patients with hepatic impairment. The dose should be reduced in this category of patients.

Administration in patients with impaired renal function

Caution should be exercised when using in patients with renal insufficiency. The dose should be reduced in this group of patients.

Administration in children

Caution should be exercised when used in children.

Indications for active substances of Retin-A (Tretinoin)

For external use: acne vulgaris (including with formation of comedones, papules, pustules), pustular acne. Favre-Racuchaux disease (nodular cystic elastosis of the skin with comedones).

Dosing regimen

 The optimal dosing regimen is determined by the doctor. It should be strictly observed in accordance with the dosage form used by a particular drug indications for use and the dosing regimen.

The antitumor therapy regimen is established individually, depending on the indications and stage of the disease.
