Precose (Acarbose)

Pharmacological action

Oral hypoglycemic drug. Competitive inhibitor of intestinal alpha-glucosidases. Inhibits the process of biotransformation of sucrose and starch to well-absorbed monosaccharides, thus reducing postprandial hyperglycemia and reducing daily blood glucose fluctuations. Unlike sulfonylurea derivatives, it has no stimulating effect on the pancreas.

Pharmacokinetics

About 35% of the administered dose is absorbed as metabolites, less than 2% - in active form. Bioavailability is 1-2%. Metabolized in the gastrointestinal tract mainly by intestinal bacteria and only partially by digestive enzymes to form at least 13 compounds as sulfate, methyl and glucurone conjugates.


One metabolite, a product of the cleavage of the glucose molecule in acarbose, has the ability to inhibit alpha-glucosidase. After oral administration, there are 2 plasma Cmax peaks of acarbose, averaging 52.2±15.7 µg/L and 586.3±282.7 µg/L and observed after 1.1±0.3 h and 20.7±5.2 h, respectively. The appearance of the second peak is apparently due to the absorption of products of bacterial decomposition from the deep parts of the small intestine. Cmax in patients with renal insufficiency (CK < 25 ml/min) is 5 times higher, in elderly patients - 1.5 times higher. Vd is 0.32 l/kg. T1/2 of acarbose in the distribution phase is 3.7±2.7 h, in the excretion phase - 9.6±4.4 h, respectively; 51% of active substance is excreted through the intestine; 1.7% unchanged and as an active metabolite and 34% as metabolites - by kidneys.

Contraindications for use

Chronic intestinal diseases with marked indigestion and absorption disorders; states accompanied with flatulence (Remheld syndrome, large hernias, intestinal stenosis, intestinal ulcers); severe renal failure (CK <25 ml/min); pregnancy, lactation (breast-feeding); age under 18 years; hypersensitivity to acarbose.


Administration during pregnancy and lactation

Contraindications for use during pregnancy and lactation.

Administration in patients with liver dysfunction

Contraindication: liver dysfunction. It is recommended to monitor the liver transaminase activity in plasma during long-term (more than 6 months) administration of acarbose at a dose of 600 mg/day.

Administration in children

Contraindication: age under 18 years.

Indications of active substance Precose (Acarbose)

Treatment of type 2 diabetes in combination with diet therapy (as monotherapy or in combination with other hypoglycemic preparations, including insulin).


Prevention of type 2 diabetes in patients with impaired glucose tolerance in combination with diet and exercise.


Dosing regimen

For oral administration, the initial dose is 50 mg 3 times a day; further, depending on the treatment regimen and the patient's individual response, the dose may be increased to 100-200 mg 3 times a day. The treatment is prolonged.
