Phenergan (Promethazine)

Pharmacological action

Blocker of histamine H1-receptors, has high antihistamine activity and a pronounced blocking effect on the CNS. Causes a sedative effect and has an antiemetic effect. It also has a pronounced adrenoblocking, moderate peripheral and central cholinoblocking effect. The effect begins to develop 20 minutes after intake and lasts for 6 hours, sometimes up to 12 hours.


After oral administration it is rapidly and well absorbed from the gastrointestinal tract.

Binding to plasma proteins is more than 90%. It penetrates through the HEB and the placental barrier.

It is metabolized in the liver to form mainly sulfoxides and N-demethylated promethazine derivatives.

After oral administration the T1/2 is 7-12 hours. It is excreted mainly by the kidneys; to a lesser extent by the intestine.

Contraindications to use

Concomitant use of MAO inhibitors, comatose state (including intoxication with alcohol), severe liver function abnormalities, first trimester of pregnancy, lactation (breast-feeding), early childhood under 2 months, hypersensitivity to promethazine or other phenothiazine derivatives.

Usage during pregnancy and lactation

Prometazine is contraindicated for use in the first trimester of pregnancy. If therapy is necessary for clinical indications in the third trimester of pregnancy, Prometazine is used in minimum maintenance doses, reducing them towards the end of pregnancy to prevent development of drowsiness or psychomotor agitation in the neonate.

If it is necessary to use promethazine during lactation, the decision should be made whether to stop breastfeeding.

Use in patients with hepatic impairment

Caution: severe hepatic impairment is contraindicated.

Administration in children

Contraindicated in children under 2 months of age.

Caution in children with acute or chronic respiratory diseases because promethazine suppresses the cough reflex.

It is not recommended for use in children to stop episodic vomiting of unspecified genesis.

Indications of the active ingredient Phenergan (Promethazine)

  • Allergic diseases and reactions: rash, itching, eczema, medicated rash, serum sickness, Quincke's edema, hay fever, anaphylactic shock, bronchoobstructive syndrome (as part of complex therapy).

  • Skin diseases (including eczema, neurodermatitis, diathesis, contact dermatitis and toxicoderma).

  • In anesthesiology: as a sedative before and after surgery, as an aid in conjunction with analgesic drugs, to prevent and treat nausea and vomiting during anesthesia and surgery, with artificial hibernation.

  • As part of combined therapy or as an additional agent: kinetosis, migraine, sleep disorders, states of agitation; gastric and duodenal ulcers.

  • In pediatrics: changes of skin and mucous membranes in infants against exudative diathesis, eczema, asthmatic bronchitis, false croup, various types of allergic dermatosis, serum sickness.

Dosing regimen

When administered orally in adults the daily dose is 75-100 mg.

Maximal doses: when administered orally, single dose is 75 mg, daily dose is 500 mg; when administered intravenously, single dose is 50 mg, daily dose is 250 mg.

V/v is used as part of lytic mixtures.

Orally in children aged 1 to 2 years - 5-10 mg 1-2 times daily, 2 to 5 years - 5-10 mg 1-3 times daily, 5 to 10 years - 5-15 mg 1-3 times daily, over 10 years and adolescents 5-20 mg 1-3 times daily. When administered intravenously, the dose is 0.5-1 mg/kg and the frequency of administration is 3-5 times/day. In emergency cases (asthmatic status, false croup) the dose may be increased to 1-2 mg/kg. When administered intravenously (usually as part of a lytic cocktail), the dose is 1/3 of the dose for intravenous administration.
