Drug title
Naprosyn (Naproxen)
Pharmacological action
NSAID, a derivative of naphthylpropionic acid. It has anti-inflammatory, analgesic and antipyretic effects.
The mechanism of action is associated with inhibition of COX enzyme, which leads to inhibition of prostaglandin synthesis from arachidonic acid.
Inhibits platelet aggregation.
Reduces pain syndrome, including joint pain at rest and on the move, morning stiffness and joint swelling, helps to increase range of motion. Anti-inflammatory effect comes by the end of the 1st week of treatment.
Pharmacokinetics
After oral administration naproxen is quickly absorbed from the gastrointestinal tract. Cmax in plasma is reached after 1-4 hours. Food slows the rate of absorption, but does not reduce the degree of absorption. Naproxen is well absorbed by rectal administration, but absorption is slower than when administered orally.
In therapeutic concentrations naproxen binds to plasma proteins by more than 99%. The plasma concentration of naproxen increases proportionally at doses up to 500 mg, at higher doses there is an increase in clearance due to saturation of plasma proteins.
Naproxen diffuses into the synovial fluid.
T1/2 is approximately 13 h. Approximately 95% of the dose is excreted in the urine as unchanged substance and 6-O-desmethylnaproxen and its conjugates. Less than 5% is excreted with the feces.
Contraindications to use
Gastrointestinal erosive and ulcerative lesions in acute phase, "aspirin triad", bleeding disorders, expressed renal dysfunction (CKR less than 20 ml/min), expressed liver dysfunction, children under 12 years old; hypersensitivity to naproxen and other NSAIDs.
Administration during pregnancy and lactation
Caution is used in pregnancy (especially in the I and III trimesters) and during lactation.
Naproxen penetrates the placental barrier and is excreted with breast milk in small amounts.
Application in liver function disorders
It is contraindicated in cases of marked liver dysfunction.
Usage in impaired renal function
Significant renal impairment (CKG < 20 ml/min) is contraindicated.
Administration in children
Contraindicated in children under 12 years of age.
Indications for the active ingredient Naprosyn (Naproxen)
Rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, joint syndrome in acute gout; spinal pain, neuralgia, myalgia, traumatic inflammation of soft tissue and musculoskeletal system. As an adjunctive agent in infectious inflammatory diseases of ENT organs, adnexitis, primary dysmenorrhea, headache and toothache.
Dosing regimen
It is established individually, taking into account the severity of the disease.
Orally in adults - 0.5-1 g/day in two doses. Daily dose for maintenance treatment is 500 mg.
Maximum daily dose is 1.75 g.
