Myambutol (Ethambutol)

Pharmacological action

It is a chemotherapeutic agent with bacteriostatic effect on typical and atypical mycobacteria of tuberculosis.


The mechanism of action of the drug is associated with rapid penetration into the cell, where it disrupts lipid metabolism, RNA synthesis, binds magnesium and copper ions, disrupts the structure of ribosomes and protein synthesis in bacterial cells.


It affects both intracellular and extracellular bacterial species. About 1% of patients have primary resistance to the drug. Etambutol is well absorbed in lung tissue and can reach concentrations 5-9 times higher than in blood serum; it penetrates well into many tissues and organs.


Intracellular concentration in erythrocytes is twice as high as in serum.


Pharmacokinetics

Etambutol is rapidly and 80% absorbed from the digestive tract. The minimum inhibitory concentration is 1mg/ml.


After oral administration of a single dose of 25 mg/kg body weight, the Cmax in serum is 2-5 mcg/ml after 2-4 h, after 24 h the concentration is less than 1 mcg/ml.


It is 20-30% bound to plasma proteins.


Etambutol is metabolized in the liver to dicarboxylic acid derivatives.


T1/2 is 3-4 h, and in renal insufficiency is prolonged to 8 h. Within 24 hours, more than 50% of the drug dose is excreted unchanged in the urine, and 8-15% is excreted as inactive metabolites. About 20-22% of the initial dose of the drug is excreted unchanged in the feces.


Etambutol penetrates through the placenta. In fetal blood ethambutol concentration is about 30% of the drug concentration in maternal blood.


Contraindications to use

  • inflammation of the optic nerve;
  • cataract;
  • diabetic retinopathy;
  • inflammatory eye disease;
  • Severe renal insufficiency;
  • gout:
  • pregnancy;
  • lactation,
  • children under 13 years of age;
  • hypersensitivity to the drug.

Use in patients with impaired renal function

Contraindication: severe renal failure. In renal diseases, the drug dose depends on the degree of renal failure, which is indicated by CK.

Administration in children

Contraindication: Children under 13 years of age.

Indications of the drug Myambutol (Ethambutol)

  • pulmonary tuberculosis;
  • extrapulmonary tuberculosis.

Dosing regimen

Adults - initial treatment period: 15 mg/kg body weight/day - single dose; continuous treatment: usually 20 mg/kg body weight/day.


The dose can be increased to 30 mg/kg body weight/day (but no more than 2.0 g) in the initial period of treatment, in case of disease relapse, in case of Koch bacillus resistance to other anti-tuberculosis drugs.


In children from 13 years old it is prescribed at the rate of 15-25 mg/kg of body weight (but not more than 1.0 g).


Complete course of treatment lasts 9 months.


In case of kidney disease, the drug dose depends on the degree of renal failure, the indicator of which is CK.


In patients who previously took drugs with tuberculostatic action, bacterial resistance develops more frequently. In such cases, ethambutol should be taken with at least one or two antituberculosis drugs that the patient has not previously taken and for which bacterial resistance has not been noted.


Isoniazid, para-aminosalicylic acid (PASC), streptomycin, cycloserine, pyrazinamide and ethionamide are used in combination therapy with ethambutol.
