Minipress (Prazosin)
Pharmacological action
Peripheral vasodilator. It is a blocker of postsynaptic α1-adrenoreceptors, prevents the vasoconstrictor effect of catecholamines. Reduces BP by decreasing RAS, reduces post-load on the heart. Dilates peripheral veins, also reducing the preload on the heart. Improves systemic and intracardiac hemodynamics in patients with chronic heart failure. Reduces the pressure in the small circle of the circulation. Vasodilator effect of prazosin is accompanied with moderately pronounced reflex tachycardia. When prescribed for a long time it reduces the concentration of atherogenic lipids in blood.
Reduces myocardial oxygen demand, increases blood stroke volume during exercise. Reduces renal vascular resistance.
Reduces resistance and pressure in the urinary tract (due to relaxation of the smooth muscles of the bladder neck, prostate gland and its capsule), which leads to a decrease in the severity of symptoms of urinary tract dysfunction.
Maximum hypotensive effect after a single dose develops in 1-4 hours, the duration of action is up to 10 hours. Both early (3-5 days) and late (by 12 months) tolerance may develop, which requires increasing the dose.
Pharmacokinetics
Prazosin is rapidly absorbed from the gastrointestinal tract, Cmax is reached within 1-3 h. Food intake has no effect on absorption. Bioavailability is 50-85%. Binding to plasma proteins is more than 95%. Metabolized in the liver through methyl group cleavage and conjugation to form four active metabolites, which account for 25% of the pharmacological activity.
T1/2 is 2-4 h, in chronic renal insufficiency and elderly patients T1/2 is increased. It is excreted mainly with bile.
Contraindications to use
Hypersensitivity to prazosin; arterial hypotension, chronic heart failure due to constrictive pericarditis, cardiac tamponade, heart defects with reduced left ventricular filling pressure; pregnancy, lactation (breast-feeding); children under 12 years of age.
Use in pregnancy and lactation
Prazosin is contraindicated in pregnancy and during lactation (breastfeeding).
Usage in children
Prohibited for use in children under 12 years of age.
Indications of the active substance Minipress (Prazosin)
Arterial hypertension, chronic heart failure (in combined therapy), Raynaud's syndrome and disease; pheochromocytoma, peripheral vasospasm caused by ergot alkaloids poisoning.
Prostate hyperplasia.
Dosing regimen
It is established individually, depending on indications, clinical situation and patient's age.
The average therapeutic dose is 4-6 mg/day. Maximum daily dose is 20 mg.
