Imuran (Azathioprine)

Pharmacological action

Immunosuppressant from antimetabolite group. Being a structural analogue of adenine, hypoxanthine and guanine that are part of nucleic acids, Azathioprine blocks cell division and tissue proliferation.


The immunosuppressive effect of Azathioprine is mainly directed to delayed hypersensitivity reactions and cellular cytotoxicity. It suppresses the reaction of tissue incompatibility. To a lesser extent acts on antibody synthesis. Compared with mercaptopurine it has a more pronounced immunosuppressive effect, with less cytostatic activity.

Pharmacokinetics

After oral administration, Azathioprine is well absorbed from the gastrointestinal tract. Protein binding is low (about 30%). In the body it is biotransformed to form 6-mercaptopurine and 6-thioinosinoic acid (active metabolites). The active substance penetrates tissues in low concentrations; the lowest amount of azathioprine is detected in the brain. T1/2 for azathioprine and its active metabolites is about 5 h. Cmax in blood plasma is reached 1-2 hours after oral administration. It is eliminated almost completely through the liver with bile, through the kidneys - not more than 1-2%.

Contraindications to use

  • pregnancy;
  • Hypersensitivity to azathioprine and/or mercaptopurine.

Use during pregnancy and lactation

During treatment, women of childbearing age should use reliable methods of contraception. If it is necessary to use during lactation, breastfeeding should be discontinued.

Indications of the drug Imuran (Azathioprine)

  • Prophylaxis of transplant rejection reactions (as part of combination therapy);
  • rheumatoid arthritis;
  • Chronic active hepatitis;
  • systemic lupus erythematosus;
  • nonspecific ulcerative colitis;
  • dermatomyositis;
  • myasthenia gravis;
  • periarteritis nodosa;
  • vulgar vesicular disease;
  • autoimmune glomerulonephritis;
  • Idiopathic thrombocytopenic purpura;
  • psoriasis.

Dosing regimen

It is established individually, taking into account the indications, the severity of the disease, the dosage of concomitantly administered drugs.


For prevention of transplant rejection reactions, a shock dose of up to 5 mg/kg orally in 2-3 doses for 1-2 months is used in combination therapy (usually together with cyclosporine and corticosteroids). Then, maintenance therapy of 1 mg/kg to 4 mg/kg orally is prescribed for a long period of time (several years). If the drug is withdrawn, the dose should be reduced gradually. In case of signs of transplant rejection, the daily dose is increased again to 4 mg/kg.


In other diseases 1.5-2 mg/kg/day in 3-4 doses are usually prescribed.


If necessary, the daily dose can be increased to 200-250 mg in 2-4 doses. The duration of the treatment course is determined individually.


In rheumatoid arthritis - 1-2.5 mg/kg/day in 1-2 doses. The course of treatment is at least 12 weeks. Maintenance dose is 0.5 mg/kg 1 time/day.


In chronic active hepatitis the daily dose is 1-1.5 mg/kg.
