Metabolized by the cytochrome P450 system. The main metabolites are L-methamphenamine, L-amphenamine and N-demethylselegiline. T1/2 - 26 ± 11 hours. It is excreted in the urine in the form of L-methamphenamine (20-63%), L-amphenamine (9-26%), N-demethylselegiline (1%) and with feces (15% during 72 h).

It is rapidly and almost completely absorbed from the gastrointestinal tract. Cmax is reached after a short time. It binds to plasma proteins by 94%. Easily passes histohematological barriers, including the BBB, and accumulates in lipid-rich tissues.

Dosing and Administration of Emsam

Inside, during meals, with monotherapy: 5 mg 2 times a day; in combination therapy with levodopa, the initial dose is 5-10 mg 1 or 2 times a day (morning and afternoon), then after 2-3 days the dose of levodopa is reduced by 10-30%.

Inside, without chewing, with a small amount of liquid.

Emsam 5 mg: 1-2 tablets. in the morning (after breakfast) or 1 table. after breakfast and 1 table. after lunch.

Emsam 10 mg: 1 tab. in the morning (after breakfast). The maximum dose is 10 mg.

In combination therapy with levodopa, the dosage of the latter can be reduced by 10-30% in the first 2-3 days.

Overdose

Symptoms: dizziness, headache, psychomotor agitation, convulsions, heart rhythm disturbances, arterial hypertension, hyperthermia, profuse sweating, respiratory depression.

Treatment: induction of vomiting, gastric lavage, the use of activated carbon; with excitement and convulsions in / in the introduction of diazepam; symptomatic therapy.

Interaction

Strengthens and lengthens the effect of levodopa, amantadine, anticholinergics, catecholamines, alcohol. Tricyclic antidepressants potentiate side effects.

Incompatible with fluoxetine, sympathomimetics, drugs that depress the central nervous system, alcohol. The effectiveness is increased by levodopa (in combination with peripheral decarboxylase inhibitors). Amantadine and anticholinergics increase the frequency of adverse reactions.

Contraindications
