Product description

Pharmacological action of Eldepryl

Buy Eldepryl in Canada

Antiparkinsonian agent. Selective inhibitor of MAO-B, involved in the metabolism of dopamine and other catecholamines. It inhibits the metabolism of dopamine, its reuptake at the level of presynaptic endings, thereby contributing to an increase in its concentration in the nuclei of the extrapyramidal system and other parts of the brain.

A single dose of 5 mg of the drug inhibits 50% of MAO in the central nervous system. The enzyme recovery period is 14 days. In therapeutic doses, it does not affect the intestinal MAO and does not interfere with the breakdown of tyramine that has entered the body with food.

Indications for use Eldepryl

Parkinson's disease treatment with Eldepryl

Parkinson's disease, symptomatic parkinsonism, additional treatment for patients with Parkinson's disease, who, against the background of prolonged use of levodopa, have a decrease in the effectiveness of therapy - akinesia (in the night or early morning period).

How to use: dosage and course of treatment

Inside (without chewing, drinking a small amount of liquid). With monotherapy - 5 mg 1-2 times a day. When taken 1 time a day - in the morning before breakfast, when taken 2 times - before breakfast and before bedtime.

In combination therapy with levodopa and carbidopa - 5-10 mg each, which allows to reduce the dose of levodopa by 10-30% per day without impairing the patient's motor functions.

Special instructions

In the treatment of patients with parkinsonism in combination with levodopa, the number and severity of side effects of levodopa are less than with monotherapy with levodopa.

When used simultaneously with sympathomimetic amines, the dose of selegiline should not exceed 10 mg / day.

Care must be taken in patients with diabetes mellitus, since MAO inhibitors can lead to an increase and prolongation of hypoglycemia.

It should be borne in mind that in case of an overdose of the drug, its selective effect in relation to MAO-B is lost: when taken in doses of 30-60 mg and the use of certain varieties of cheese and other foods containing tyramine and other biogenic amines (bread with bran, sauces containing enzymes and soy, beer, wine with low vol.% ethanol, cherry liqueur, overripe fruits, smoked or pickled meat, poultry, fish, sauerkraut, chocolate), as a result of the simultaneous inhibition of MAO-A and inhibition of the first-pass effect "adrenostimulants through the liver may develop their systemic action, which is manifested by attacks of headache and increased blood pressure.

It is excreted in the form of metabolites that have an amphetamine-like effect.

treatment Fluoxetine can be started no earlier than 14 days after discontinuation of selegiline.

During treatment, patients should refrain from engaging in potentially hazardous activities that require increased attention and speed of mental and motor reactions.

Interaction

Increases the effects of levodopa, ethanol, increases the risk of side effects of amantadine.

Increases the effect (mutually) of adrenostimulants and drugs that depress the central nervous system.

Incompatible with opioid analgesics, nonspecific MAO inhibitors.

When combined with fluoxetine, the risk of developing "serotonin syndrome", accompanied by mania, hyperhidrosis, fever and increased blood pressure, increases.

Indirect and mixed adrenostimulants increase the risk of a pronounced increase in blood pressure.

Contraindications

Hypersensitivity, extrapyramidal disorders not associated with dopamine deficiency (essential tremor, Huntington's chorea), concomitant use of antidepressants and a period up to 5 weeks after their cancellation, pregnancy, lactation. With caution. Progressive dementia, severe psychosis, tardive dyskinesia, coarse tremor, peptic ulcer of the gastrointestinal tract (including history), thyrotoxicosis, pheochromocytoma, prostatic hyperplasia (with residual urine), diffuse angle glaucoma, angina pectoris, severe zoster , age up to 18 years (experience in clinical use is not enough).

When combined with levodopa: open-angle glaucoma (chronic course), hepatic and / or renal failure, melanoma (including a history), CNS depression, seizures, bronchial asthma, pulmonary emphysema, myocardial infarction (incl. in history, as well as in combination with atrial, nodal or ventricular arrhythmias), age up to 12 years (safety and efficacy have not been established).

Side effects

From the digestive system: decreased appetite, nausea, constipation or diarrhea, dry mouth, dysphagia, increased activity of "hepatic" transaminases.

From the nervous system: increased fatigue, dizziness, headache, anxiety, motor and mental agitation, insomnia, hallucinations, dyskinesia; rarely - confusion, psychosis.

From the CVS: sometimes - orthostatic hypotension, rarely - increased blood pressure, arrhythmia.

From the senses: impaired visual acuity, diplopia.

From the urinary system: urinary retention, rarely - painful urge to urinate, nocturia.

Allergic reactions: skin rash, shortness of breath, bronchospasm, photosensitivity.

Others: sometimes - hypoglycemia, hair loss, hyperperspiration. Overdose. Symptoms: when used in doses exceeding 60 mg / day, MAO-A is inhibited, which with the simultaneous use of tyramine-containing foods can provoke headaches, dizziness, increase or decrease in blood pressure; insomnia or drowsiness, weakness, anxiety, chest pain, opisthotonus, trismus, tachycardia, arrhythmia, fever, confusion, shock.

Treatment: symptomatic, no specific antidotes.
