Elavil (Amitriptyline)

Pharmacological action

Antidepressant from the group of tricyclic compounds, dibenzocycloheptadine derivative.


The mechanism of antidepressant action is associated with increased concentration of noradrenaline in synapses and/or serotonin in CNS due to inhibition of neuronal reuptake of these mediators. With long-term use, it reduces the functional activity of β-adrenoreceptors and serotonin receptors of the brain, normalizes adrenergic and serotonergic transmission, restores the balance of these systems that are disturbed in depressed states. In anxiety-depressive states it reduces anxiety, agitation and depressive manifestations.


It also has some analgesic effect, which is believed to be associated with changes in monoamine concentrations in the CNS, especially serotonin, and influence on endogenous opioid systems.


It has a pronounced peripheral and central anticholinergic effect due to its high affinity for m-cholinoreceptors; a strong sedative effect associated with affinity for histamine H1-receptors, and an alpha-adrenoblocking effect.


It has antiulcer effects, the mechanism of which is caused by its ability to block histamine H2-receptors in the parietal cells of the stomach; it also has sedative and m-cholin-blocking effects (in case of gastric and duodenal ulcer it reduces the pain syndrome and promotes acceleration of ulcer healing).


Its efficacy in nocturnal urinary incontinence is apparently due to its anticholinergic activity, which leads to increased ability of the bladder to stretch, direct β-adrenergic stimulation, α-adrenoreceptor agonist activity, accompanied by increased sphincter tone and central blockade of serotonin uptake.


The mechanism of therapeutic action in bulimia nervosa has not been established (possibly similar to that in depression). Amitriptyline has been shown to be clearly effective in bulimia in patients both without and with depression, and a decrease in bulimia may be observed without concomitant attenuation of depression itself.


In general anesthesia, it reduces BP and body temperature. It does not inhibit MAOI.


Antidepressant effect develops within 2-3 weeks after the start of use.

Contraindications to use

Acute period and early recovery period after myocardial infarction, acute alcohol intoxication, acute intoxication with hypnotics, analgesics and psychotropic agents, closed angle glaucoma, severe AV- and intraventricular conduction disorders (Guis Bundle Blockade, AV-blockade of II degree), lactation, children under 6 years of age (for oral administration), children under 12 years of age (for IV and IV administration), concomitant treatment with MAO inhibitors and 2 weeks before their use, hypersensitivity to amitriptyline.

Use in pregnancy and lactation

Amitriptyline should not be used in pregnancy, especially in the first and third trimesters, unless absolutely necessary. Adequate and strictly controlled clinical safety studies of amitriptyline use in pregnancy have not been conducted.


Amitriptyline should be phased out at least 7 weeks before expected delivery to avoid the development of withdrawal syndrome in the newborn.


In experimental studies, amitriptyline had teratogenic effects.


It is contraindicated during lactation. It is excreted with breast milk and may cause drowsiness in infants.

Administration in children

Contraindication: children under 6 years of age (for oral administration), children under 12 years of age (for intravenous and intravenous administration).

Indications for the active ingredient Elavil (Amitriptyline)

Depression (especially with anxiety, agitation and sleep disorders, including in childhood, endogenous, involutional, reactive, neurotic, drug-induced, in organic brain lesions, alcohol withdrawal), schizophrenic psychoses, mixed emotional disorders, behavior disorders (activity and attention), nocturnal enuresis (except for patients with bladder hypotension), bulimia nervosa, chronic pain syndrome (chronic pain in cancer patients, migraine, rheumatic pain, atypical facial pain, postherpetic neuralgia, posttraumatic neuropathy, diabetic neuropathy, peripheral neuropathy), migraine prevention, gastric and duodenal ulcer.

Dosing regimen

For oral administration, the initial dose is 25-50 mg at night. Then, within 5-6 days, the dose is increased individually to 150-200 mg/day (most of the dose is taken at night). If during the second week, there was no improvement, the daily dose is increased to 300 mg. If symptoms of depression disappear, the dose is reduced to 50-100 mg/day and the therapy is continued for at least 3 months. In elderly patients with mild disorders, the dose is 30-100 mg/day, usually once a day at night; after therapeutic effect is achieved, the minimum effective dose is changed to 25-50 mg/day.


In nocturnal enuresis in children aged 6-10 years, 10-20 mg/day at night; in children aged 11-16 years, 25-50 mg/day.


In/m - the initial dose is 50-100 mg/day in 2-4 injections. If necessary, the dose may be gradually increased to 300 mg/day, in exceptional cases to 400 mg/day.
