Clindamycin phosphate is inactive in vitro, but is rapidly hydrolyzed in vivo to form clindamycin, which has antibacterial activity. Clindamycin inhibits protein synthesis in a microbial cell by interacting with the 50S ribosome subunit, has a bacteriostatic effect, and in higher concentrations against some microorganisms is bactericidal.

Pharmacokinetics

Cleocin appearance

In healthy volunteers after intravaginal application of 5 g of clindamycin phosphate cream (clindamycin 20 mg / g), Cmax of clindamycin averaged 20 ng / ml (range from 3 to 93 ng / ml). Approximately 3% (0.1 to 7%) of the administered dose is absorbed systemically.

Systemic use: infectious diseases caused by sensitive microorganisms: respiratory infections (pneumonia, including aspiration, bronchitis, lung abscess, fibrosing alveolitis, pleural empyema), infections of ENT organs (including pharyngitis, sinusitis, tonsillitis, otitis media) , infections of joints and bones (septic arthritis, osteomyelitis), purulent infections of soft tissues and skin (including acne, phlegmon, boils, impetigo, infected wounds, panaritium, abscesses, erysipelas), septicemia (mainly anaerobic), intra-abdominal infections and infections pelvic organs (including peritonitis, abscesses of the abdominal organs, only under the condition of the joint use of drugs that are active against gram-negative aerobic microorganisms), infections of the oral cavity (including periodontal abscess), gynecological diseases (including endometritis, abscesses of the fallopian ovarian tubes, adnexitis , pelvioperitonitis, salpingitis), tropical malaria (which causes van Plasmodium falciparum and resistant to chloroquine without or in combination with quinine), toxoplasmic encephalitis, sepsis, pneumocystis pneumonia (caused by Pneumocystis carinii), scarlet fever, bacterial endocarditis, diphtheria.



Contraindications for use

Application during pregnancy and lactation

When using clindamycin intravaginally in the II and III trimester of pregnancy, there was no increase in the frequency of congenital fetal anomalies. If Clindamycin vaginal cream is used during the II and III trimesters of pregnancy, then an adverse effect on the fetus seems unlikely.

Adequate controlled studies on the use of the drug in the first trimester of pregnancy have not been carried out, therefore Clindamycin vaginal cream can be prescribed to women in the first trimester of pregnancy only for absolute indications, i.e. when the potential benefit of drug therapy outweighs the potential risk to the fetus.

It is not known whether clindamycin is excreted in breast milk after intravaginal use. Clindamycin is found in breast milk after oral or parenteral administration, therefore, when deciding on the possibility of prescribing clindamycin in the form of a vaginal cream during breastfeeding, the expected potential benefits of drug therapy and the possible risk to the child should be assessed.

Special instructions

Before prescribing the drug, using appropriate laboratory methods, Trichomonas vaginalis, Chlamydia trachomatis, Neisseria gonorrhoeae, Candida albicans and Herpes simplex, which often cause vulvovaginitis, should be excluded.

Intravaginal use of clindamycin can lead to increased growth of insensitive microorganisms, especially yeast-like fungi.

Given the possibility of minimal systemic absorption of the drug (approximately 4%), the occurrence of diarrhea during the use of the drug cannot be excluded. In this case, the drug should be canceled.

Patients should be warned that during drug therapy they should not have sexual intercourse, as well as use other means for intravaginal administration (tampons, douching).

The drug contains components that can reduce the strength of latex or rubber products, therefore, the use of condoms, vaginal contraceptive diaphragms and other latex products for intravaginal use during drug therapy is not recommended.

Influence on the ability to drive vehicles and use mechanisms

There is no reason to believe that the use of the drug Clindamycin vaginal cream may affect the ability to drive a car and use mechanisms.

Overdose

Clindamycin vaginal cream is intended only for intravaginal use.

No drug overdose was reported. Accidental ingestion of the drug into the gastrointestinal tract can cause systemic effects similar to those that occur after oral administration of clindamycin in therapeutic doses.

Treatment: symptomatic and supportive.

Drug Interactions

There is cross-resistance between clindamycin and lincomycin. Antagonism between clindamycin and erythromycin has been demonstrated under in vitro conditions.

No interactions were observed in pharmacodynamic, pharmacokinetic or clinical studies of this dosage form.

There is no information on the use of other drugs in conjunction with the drug. Combined use with other drugs for intravaginal administration is not recommended.

Side effect

The incidence of the following side effects is less than 10%.

From the urinary system: vaginal candidiasis, irritation of the mucous membrane of the vulva and vagina, vulvovaginitis, Trichomonas vaginitis, vaginal infections, menstrual irregularities, pain in the vagina, uterine bleeding, dysuria, vaginal discharge, urinary tract infections, abnormal labor, endometriosis, glucosuria , proteinuria.
