I can hardly believe that I spent so much money on brand drugs. Your generic drugs act the same way the brand ones do but they cost much more cheaper. Thank you. Now I know about your site and will buy again and again.

Product description

Pharmacokinetics of Bystolic

Buy Bystolic in Canada

After oral administration, it is rapidly absorbed from the gastrointestinal tract (food intake does not affect absorption). Bioavailability - 12% in persons with "fast" (the effect of "first pass" through the liver) metabolism and almost complete - in people with "slow". The protein binding for D-nebivolol is 98.1%, for L-nebivolol - 97.9%. It is metabolized to form active metabolites by acyclic and aromatic hydroxylation and partial N-dealkylation; the resulting hydroxy and amino derivatives are conjugated with glucuronic acid and excreted in the form of O- and N-glucurones. T1/2 of hydroxymetabolites - 24 hours, nebivolol enantiomers - 10 hours (for persons with a "fast" metabolism); for persons with "slow" - 48 hours and 30-50 hours, respectively. It is excreted by the kidneys (38%) and intestines (48%).

Bystolic Tablets Appearance

The combination with anesthetic substances causes a cardiodepressant effect and increases the risk of arterial hypotension. With simultaneous use with antihypertensive drugs, nitroglycerin, blockers of "slow" calcium channels, severe arterial hypotension may develop (special care should be taken when combined with prazosin); with antiarrhythmics, reserpine, alpha-methyldopa, clonidine, guanfacine - an increase in the severity of bradycardia; with cardiac glycosides - summation of the chrono- and dromotropic effect; with clonidine - increased withdrawal syndrome.

Inductors of microsomal oxidation (rifampicin, barbiturates) decrease, and inhibitors (cimetidine) increase the concentration in blood plasma.

Method of administration and dosage of Bystolic

Inside, regardless of food intake, with a sufficient amount of liquid, at the same time of day. The average daily dose is 2.5–5 mg. Take 1 time per day. Patients with renal insufficiency or patients over 65 years of age are recommended in an initial dose of 2.5 mg / day; if necessary, the daily dose can be increased to 10 mg.

Inside, at the same time of day, regardless of food intake, without chewing and drinking plenty of fluids.

Arterial hypertension and ischemic heart disease. The average daily dose for the treatment of hypertension and ischemic heart disease is 2.5–5 mg (1 / 2–1 table. 5 mg) 1 time per day. It is possible to use the drug in monotherapy or in combination therapy.

The optimal therapeutic effect becomes pronounced after 1–2 weeks of treatment, and in some cases - after 4 weeks. If necessary, the daily dose can be increased to a maximum of 10 mg (2 tablets, 5 mg per dose).

Elderly patients: for patients over 65 years of age, the initial dose is 2.5 mg / day (1/2 table, 5 mg each). If necessary, the daily dose can be increased to 5 mg. However, in view of the limited experience with nebivolol in elderly patients, care must be taken to ensure close monitoring.

Patients with renal insufficiency: the initial dose for patients with impaired renal function (Cl creatinine> 20 ml / min) is 2.5 mg / day (1/2 table. 5 mg). If necessary, the daily dose can be increased to 5 mg. There is no experience with nebivolol in patients with severe renal impairment (Cl creatinine <20 ml / min), so its use is not recommended in them. Increasing the dose in these patients should be done with extreme caution.

Chronic heart failure. Treatment of chronic heart failure should begin with a gradual increase in dose until the individual's optimal maintenance dose is reached. Patients should not have had an acute heart failure attack within the past 6 weeks. It is recommended to carry out treatment under the close supervision of a physician.

For patients taking diuretics, cardiac glycosides, ACE inhibitors, ARA II, dosing should be stabilized 2 weeks before starting nebivolol treatment.

Selection of doses in the beginning of treatment necessary to carry out the following scheme, keeping weekly or bi-weekly intervals, and based on the tolerability of that dose by the patient: the dose is 1.25 mg drug Bystolic® (1/4 Table 5 mg.) 1 times a day, can be increased first to 2.5–5 mg of Bystoliceach® (1 / 2–1 table. 5 mg), and then to 10 mg (2 table. 5 mg each) 1 time per day.

The patient should be under the supervision of a physician for 2 hours after taking the first dose of the drug, as well as after each subsequent dose increase. Each increase should be carried out at least 2 weeks later. The maximum recommended dose for the treatment of chronic heart failure is 10 mg / day in 1 dose. During dose titration, regular monitoring of blood pressure, heart rate and symptoms of CHF is recommended. During the titration phase, in case of worsening CHF or drug intolerance, it is recommended to reduce the dose of the drug or, if necessary, immediately stop taking it (in case of pronounced arterial hypotension, worsening of the course of chronic heart failure with acute pulmonary edema, the development of cardiogenic shock, symptomatic bradycardia or AV -blockades).

Treatment for stable chronic heart failure is usually long-term.

It istreatment with Bystolic® not recommended to stopabruptly, as this may lead to a temporary exacerbation of the course of chronic heart failure. If it is necessary to stop taking the drug, the cancellation is carried out gradually, reducing the dose by half within a week.

Hypersensitivity, bronchial asthma, heart failure (in the stage of decompensation), severe liver dysfunction, arterial hypotension, severe bradycardia, cardiogenic shock, sick sinus syndrome, II and III degree AV block, pheochromocytoma, Prinzmetal angina pectoris, depression, obliterating diseases peripheral vessels ("intermittent" claudication), myasthenia gravis, muscle weakness, age up to 18 years (efficacy and safety have not been established).

Application during pregnancy and lactation, fertility

In pregnancy and lactation should be used with caution, according to strict indications, under medical supervision.

Symptoms of an overdose of beta-blockers: lowering blood pressure, nausea, vomiting, cyanosis, sinus bradycardia, AV blockade, heart failure, cardiogenic shock, cardiac arrest, bronchospasm, loss of consciousness, coma.

In clinical practice, overdose phenomena were not observed.

Treatment: gastric lavage, the appointment of activated charcoal, in case of severe hypotension, bradycardia and heart failure - intravenous administration of beta-adrenostimulants with an interval of 2-5 minutes until the desired effect is achieved. In the absence of effect - dopamine, dobutamine, norepinephrine. Then 1-10 mg of glucagon, the setting of a transvenous intracardiac stimulator. With bronchospasm - intravenous administration of betastimulants2-adrenoceptor. With ventricular premature beats - lidocaine (class IA drugs are not used). For convulsions - intravenous diazepam.
