Chloroquine (Aralen)

Pharmacological action

Antimalarial medicine. By inhibiting the synthesis of nucleic acids, it has a moderately pronounced immunosuppressive and nonspecific anti-inflammatory effect. Causes death of asexual erythrocytic forms of all types of plasmodia. It has gametocidal action, except for Plasmodium falciparum (shows antigametocidal action).


After oral administration, it is quickly and almost completely absorbed from the gastrointestinal tract. Cmax in blood is reached after 2-6 hours. About 55% is bound to plasma proteins. It is distributed in the body tissues. Chloroquine (Aralen) accumulates in liver, kidneys, spleen and lungs. It easily penetrates through the BBB and placental barrier and is excreted with breast milk. A small amount is metabolized (25%). It is excreted by the kidneys (70% - unchanged) slowly. T1/2 is 1-2 months.

Contraindications for use

Hypersensitivity to chloroquine; hepatic and/or renal insufficiency, suppression of medullary hematopoiesis, severe rhythmic disorders, psoriatic arthritis, neutropenia, porphyrinuria; pregnancy, lactation (breast-feeding).

Caution: glucose-6-phosphate dehydrogenase deficiency, retinopathy, epilepsy.

Administration during pregnancy and lactation

Chloroquine is contraindicated in pregnancy and during lactation (breast-feeding). If it is necessary to use during lactation, it is necessary to decide whether or not to stop breastfeeding.


Chloroquine is used with caution in glucose-6-phosphate dehydrogenase deficiency, liver and/or kidney function disorders, epilepsy.

Systematic ophthalmologic examinations and general blood tests should be performed during the treatment period.

Indications for the active ingredient Chloroquine (Aralen)

Malaria (prevention and treatment of all types), extraintestinal amoebiasis, amebic liver abscess, systemic lupus erythematosus (SLE) (chronic and subacute forms), rheumatoid arthritis, scleroderma, photodermatosis.

Dosing regimen.

Inside. The dose, regimen and duration of administration are set individually, depending on the indication and clinical situation.

Drug interaction

Co-administration of Chloroquine (Aralen)

 Co-administration of chloroquine with digoxin increases plasma concentration of digoxin; with cimetidine - a significant decrease in metabolism and excretion of chloroquine; with ampicillin - decrease in its absorption.

Concomitant use of Chloroquine (Aralen) with phenylbutazone, gold drugs, penicillamine, cytostatics, levamisole increases the risk of bone marrow aplasia and skin lesions.

Antacids impair absorption of Chloroquine (Aralen).

Combination of Chloroquine (Aralen) with other antimalarial drugs may be accompanied by antagonistic effects.

When combining with GCS, the risk of myopathy and cardiomyopathy increases, with MAO inhibitors - the risk of neurotoxicity increases, with ethanol - hepatotoxicity.
